Both parents are deceased, missing, or in a persistent vegetative state
One parent is deceased, missing, or in a persistent vegetative state, and the other parent has been convicted of a felony or an offense of violence posing a substantial threat of harm to the child’s health or welfare
The child has been adjudicated dependent and removed from a parent’s custody, which may open a separate pathway under Chapter 39
